Eligibility

Newly hired full-time employees working 30 hours or more will be eligible to enroll in health insurance benefits during the new employee orientation period. Eligibility begins on the first of the month following 30 days of hire.

Benefit Information

How do I enroll?

To enroll in benefits, all employees must submit their benefit elections through Employee Navigator

If you miss the new hire or open enrollment window, you will have to wait until the next open enrollment period to enroll in benefits unless you experience a qualifying life event.

Waiving or Declining Health Insurance

If you are eligible for health insurance but have alternative coverage or choose not to have insurance, you must sign a waiver form during the enrollment period. If you decide not to enroll, you won’t be able to join the plan until the next Open Enrollment period, unless there is a “qualifying event.” A Qualifying Life Event is a life-changing situation that allows you to make changes to your benefit elections outside of the normal Open Enrollment period. Examples of Qualifying Life Events include the birth of a child, marriage, divorce, and loss of other coverage.

Vision coverage is available with enrollment in the Anthem’s Access Blue New England, at no additional cost. Anthem Blue View Vision Plan offers a national network of providers and provider locations. Additional stand-alone vision coverage is offered through DeltaVision®, supported by the nationwide EyeMed Vision Care Access Network, which includes private practitioners as well as leading retail and online providers

Your Health Reimbursement Account Benefit

Eligibility

Employees and their eligible dependents that qualify under IRS guidelines for tax purposes and are covered under Pompanette, LLC with Anthem.

Health Reimbursement Account (HRA)

Pompanette, LLC is excited to continue our Health Reimbursement Arrangement (HRA) as part of your benefits package. The HRA is an employer benefit plan where the employer reimburses a portion of your plan’s annual deductible and coinsurance.

How does a Health Reimbursement Account work?

Employees and their eligible dependents that qualify under IRS guidelines for tax purposes are covered under Pompanette, LLC with Anthem. Reimbursements will only be made if the member incurs a deductible related medical expense during the plan year. There is no carry-over provision for this HRA plan.

Group Life and Accidental Death and Dismemberment (AD&D) Insurance

Overview of Group Life & AD&D Coverage

The group term Life and Accidental Death and Dismemberment (AD&D) insurance available through your employer gives extra protection that you and your family may need. Life and AD&D insurance offers financial protection by providing you coverage in case of an untimely death or an accident that destroys your income-earning ability. Life benefits are disbursed to your beneficiaries in a lump sum in the event of your death.
